Case Management: Discharge Planner (Casual)

Variable as scheduled as needed.
Five years previous clinical healthcare experience required.
Bachelors degree Human Services, RN, BSW, MSW preferred.
If RN , RN licensed in the State of Delaware.
Experience in Case management preferred.
Skills include ability to complete psychosocial assessment, exceptional communication and problem solving skills, general knowledge of utilization management, understanding of levels of care and community resources.
Demonstrated skills of organization, team work, strong interpersonal and written communication skills, and flexible work schedule, and be self motivated.
A discharge planner provides assistance to the care coordinator in completing referrals to other facilities or next level of care, to include final discharge arrangements, home health services, DME, or other post-discharge services.
In collaboration with the care coordinator, the discharge planner will screen patient needs to assist in discharge planning.
This includes coordination and verification of eligibility benefits for discharge needs, making copies of records, faxing documents, etc.

Estimated Salary: $20 to $28 per hour based on qualifications.
