Care Coordinator: Population Health (FT)

Full TimeTwo years minimum experience in health care- preferably primary care, case management and/or acute care.
Associates or Bachelors Degree in nursing, social work, psychology or business or related field.
Knowledge of the community, challenges in navigating the health care system, provision of patient-centered care and quality metric reporting.
Excellent communication skills and the ability to work collaboratively with others.
Computer proficiency and familiarity with Electronic Health Record documentation required.
This individual is responsible for providing care coordination services for patients of the Delmarva Health Network.

Estimated Salary: $20 to $28 per hour based on qualifications.
