Summary:
The Community Outreach Population Health Nurse Practitioner (NP) will provide primary health care and behavioral health services as a member of a healthcare team comprised of Behavioral Health and Community Outreach RNs, Peers, Community Health Workers, and driversThe Nurse Practitioner in this role will function within the appropriate scope and credentials as approved by Beebe HealthcareIn this role the NP will work on our mobile health clinic (MHC) that will travel throughout Sussex CountyThe MHC has a focus on Behavioral Health and Addiction MedicineAdditionally, preventative/primary health care and social healthcare will be included in services and care offeredThis individual must be willing to work closely with other staff and be comfortable in a mobile environment of careLocations include, but are not limited to, areas where underserved/uninsured individuals reside, homeless encampments, recovery homes, churches, schools, employer sites, and communities who are in need of servicesThe goal is to connect with patients who are unable to get to a Beebe Healthcare facility so we can bring the care to themServices include behavioral health care, preventative/primary care, vaccines, point of care testing/lab draws, and partnering with area organizations to best serve patients and their needsIncludes traveling to multiple locations, and hours can vary from morning to early afternoon or late afternoon to late eveningPosition is a casual position that has opportunity to growThis person would be the only NP on the mobile clinicThis position's primary location will be Beebe Healthcare's Community Mobile Health Clinic

Provides primary care directly on a mobile health clinic, emphasizing accessibility, health promotion, illness prevention and continuity of care to patients presenting with both minor and complex health issuesTakes a complete history including medical, psychosocial, economic, relationship and family statusCompletes physical examinations, performs/orders appropriate diagnostic testsManages common acute and chronic illnesses, including minor trauma, episodic problems, and common chronic disordersDiscusses health concerns/diagnosis with patients and families, and together determines an appropriate plan of careAdministers vaccines according to accepted guidelines and administers medications as appropriatePrescribes Medication-Assisted Treatment (MAT), including buprenorphineDevelops and reviews clinical protocols and best practice guidelinesQuality of Service:
Documents all patients encounters completely and in a timely mannerThis includes proper documentation of phone calls, prescription, and if applicable, the complete dictation of patient visits for a session prior to leaving the mobile health clinicAdheres to clinical practice guidelines that are adopted by Beebe HealthcareKeeps medical knowledge current through attendance at medical CME programs at least 20 hours per yearMaintains an acceptable level of compliance with clinical and regulatory standards including medical record documentation per Joint Commission standardsDemonstrates appropriate interactions toward patients & families in a manner that helps to put patients at easeParticipants in the Performance Improvement activities of the Beebe Healthcare and Mobile Health Clinic through chart audits, development and implementation of practice standardsTeamwork:
Works collaboratively with the multidisciplinary team to provide treatment, ongoing follow-up and evaluation of the plan of care with the patientAssists mobile health clinic staff willingly in difficult situations in the interest of providing quality service to the patientsDemonstrates constructive problem solving with colleagues and mobile clinic staffSupports established policies and procedures of the Mobile Health ClinicMakes recommendations and contributes to policy formulationStarts patient schedule on time and leaves after completing the work schedule including needed record documentationCompletes required documentation for referrals and consultationsReturns patient telephone calls within an appropriate time frame based on patient needs and expectationsMiscellaneous:
